Please
read the following carefully and return one signed
copy to the mailing address above along with your deposit of $100. A
signed copy
and payment is required prior to enrollment. In order to guarantee
enrollment,
the registration form and deposit must be received by June 1,
2009.
1. The 2009 summer
session starts June 15 and runs through August 22. This summer
semester
consists of 10 children's art classes, which may be scheduled to work
around
vacations and summer camps. For example, if you wish your child to
attend
weekly for 5 weeks, then times a week for 1 week, just sign up
for
the days
and times you prefer. If you wish to add
extra classes over the 10, you may do so. The classes will be
available on a
first come, first served basis, so schedule your child's classes as
quickly as
possible. The classes have a limited enrollment of 6 students per
class.
Available class times are Wednesday through Friday 2:30-4 or 5-6:30,
and
Saturdays
11-12:30 or 1:30-3. Fill out the form below with your preferred times
and
alternate times, and you will be contacted to confirm your enrollment
times.
2. Tuition for
the 10 day summer semester
package is $280. Additional art classes are $28 per class. If your
child is
unable to sign up for 10 classes, you may sign up for individual
classes
after
June 1 for $33 per class. All students must pay a minimum deposit of
$100 to
reserve their spot, and the remainder of their tuition is due the first
day of
class.
4. If a lesson
is missed, although Amanda
will work with you to schedule a makeup lesson, a makeup class cannot
be
guaranteed nor can credit be given for that lesson. If a class is
missed
without 24 hour notice, there will be no make-up. If a make-up lesson
is
missed, there will be no further make-ups for the missed lesson. Credit
will be
offered only when the instructor must cancel a lesson. Make-up classes cannot be carried into the next semester.
5. All drawing
supplies are provided free of charge. Students will be
responsible for the following
painting supplies when they begin their painting unit: brushes,
canvasses,
paints, palette, palette knife, and paint or tackle box. Easels,
nitrile gloves, mediums, shop towels, and various miscellaneous
materials are provided. A detailed supply list will be provided to the
student at the appropriate time.
6. Amanda Jones
reserves the right to refuse
instruction at any time for poor attendance, attitude or behavior that
is
disruptive to the class, or violation of this registration agreement.
